Community care grant form help

I have just moved into a flat after being in refuge.
I had a form for the CCG but lost it during the move and had to get another one.
I am confused as how to fill it out.
Basically It asks for your address then a little way on it asks about being resettled as part of a project, which I was, been rehomed from refuge through the council.
I'm confused as it asks ''where am I moving to'' but I have already moved, do I just put the new address in both or leave the latter blank?

not quite the same thing but i did get 600 pound, i had moved 5 months before aswell and council moved me on health grounds but under the resetrament rules, i give everdance of my prevoius area and the resetilment letter of the council and my high banding, i put the adress iam at now and got nearly everything i asked for good luck with it and if your not happy with what your given if at all appeal x0

Unless you have a package with Social Work department, you are not on a resettlement programme with the council.
A resettlement programme is a package with an organisation like Social Work, Salvation Army, Womens's aid where there is a clear action plan to "resettle" you into the community after an unsettled way of life.
Do you have a planned programme of resettlement drawn up with the refuge? If not, and if you don't have a planned programme set up with any other organisation which includes assistance and plans for dealing with with other things other than gaining a tenancy, (being rehomed is not enough in itself to constitute a planned programme of resettlement for CCG purposes, it is only a mere part of a programme), then you are not on a "resettlement programme", and should instead claim under "exceptional pressure".

Well I was in a womens aid refuge but I applied to and secured the accomodation without their help as they wouldnt help me because I didnt settle in the refuge area.
My children are on child protection register with SS due to my violent ex, but they didnt help me either, well apart from writing a letter.0 -
Exceptional pressure - setting up a family home (from scratch, I assume?) after fleeing domestic violence. Put yuor social workers details on the form too, so that they can contact them to gather information in support of your claim.0
