My brothers in trouble with his credit card

Hey guys hope you can help, my brother recieved this letter from Halifax last week. Its regarding a credit card
Arrears amount £1,376.82
Outstanding balance £3,812.85
Arranged payment £0.00
Frequency
Start date 0000
Dear Sir/Madam
The arrangement that was agreed as a concession has now expired.
The repay the arrear outstanding immediately.
Then goes onto say if you need help phone.
what i dont get is that the cc max amount was £1,200.
1) How has it got his high
2)Whats the best course of action
He has just had a kid and i struggling financially at the mo.
