Just put your own name as 'company' and 'owner' in the job title box, if you are self-employed.

A lot of people requiring safety goggles are self-employed. However, the question to ask is whether they will send free samples to self-employed people who won't have more than a couple of people helping them so won't be putting in a large order for goggles unlike a bit construction company.
