I cannot recall it happening before, but my in box contained mail to
anor@notmyisp.de
What is the explanation for this? My SPAM protection level with my ISP is set high

yes this is because the sender send multiple address at once via bcc option.0

Basically, your ISP's spam filtering didn't recognise the mail as spam.
On another level name@ username.isp.com is the To: address (note the ":"), which can be forged. It is not the envelope To (which cannot, for obvious reasons, be forged).0
