Communuity care grants, different address to send claim form to?

I am moving out of refuge into a flat on friday :j
I have had a community care grant form for ages that I got from the local JCP office.
Bascially I'm moving to a completely different area, 2 counties away, so will it be a different BDC I have to send the claim form to?
If so, can I just find out the address and sent it in a plain envelope or would it be best to just get a new claim form from the local JCP to my new flat?

Just go to the JCP website on here type in your postal code and your local JCP will come up send it too that address and good luck in your new flat
