Debenhams issue Credit Card despite cancelling

Hi,
I know there are existing threads from end of last year about Debenhams issuing credit cards (converting storecards) if the storecard user didn't do anything to stop them.
My wife got one of these letters saying they were going to do this with her storecard about three or four months ago. Fuming a little (I remember M&S got their hands rapped for doing this, but it turns out the Debs T&C allowed them), she range them up and asked them not to convert the card. So that was that... Well, not so.
This morning she has received a letter with her new PIN for her new credit card. Nicely, the number to ring up is 0870 (but saynoto0870.com gives the geographical alternative -- they're not making money out of us for their "error").
Anyone else had a definite case where they converted a card that had been requested not to? Which "OF" body should I contact about this?
