Dentist problems - Itemised Bill etc

Hi all,
I wonder if you can help me. I have been to see my dentist today and got a few fillings done and I am getting a mouthguard.
No problem for me, because I am covered by an insurance and will get the money back, if....
...And here is the problem:
Apparently the dentist wants to charge me for filling out the document. fair enough, but they also arent able to give me an itemised bill, which I find highly suspicious! I need to provide 1 or the other, if not, I wont get my money back...
Does anyone know, if I am entitled to receive an itemised bill? They can print out an itemised estimate, but no bill... Surely the program they use can provide that.
Can anyone help me, and tell me where I stand?!

Until the work is finished they can only give you an estimate not a bill as things can often change in dentistry ie a small filling turns out to be larger or needs rct etc. And so you can never be totally sure of exact costs until end of treatment.
At the end of treatment when you have paid they will have no problems giving you an itemised receipt. Insurance companies normally only pay out at end of treatment anyway .0 -
My dental practice also said that they could only print an itemised estimate, but we got round it by them dating it for the date of treatment. Along with the till receipt, which just stated 'dental treatment, and the visa slip, all were sent to insurance company (Tesco), and that was accepted.
