where does my wife stand legally

According to barclays bank my wife took out accidental death cover verbally over the phone in 1999, which she has been paying every month since.

My wife is a person who never checks her bank statement(she wishes she had now).

Now she does not remember doing this at all.

My question if it was done verbally over the phone would barclays have had to send her any paperwork to sign. She has asked the bank to send her a copy of it.

If they cant produce it can she claim the monies back she has already payed, which amounts to over £1200.
