BT problems going back 4 years on every bill, advice needed

I have had a phone line with them 4 years, pretty much every bill from the start I have been overcharged and by the time I have complained and the correct bill given its past the payment due date so I get a late payment charge and reconnection charge on following bill.
Each time I go to high level complaints and they do sort it but "investigate" first.
I also worry about my credit rating due to this but the current issue goes back as far as Decemeber, I was told all charges would be wiped and a new bill generated which I never recieved and was just told it was due to Christmas, I then forgot about stuff to get a bill 3 weeks ago saying I owed £125 which sounds wrong as the bill they claim I owed for december was like £15 and 3x about £11.50 line rental then add vat and me using off peak calls only to landlines apart from about 2 hours of 0845 numbers during the day doesnt make £125 though I think in part its because they charged me 6 months line rental but still
