Free eye test and glasses/Dental care

After working continuously for eight years I damaged my shoulder last August and have been unable to work since.
I am on ESA(had assessment waiting for verdict) although not sure which form of ESA (income,contribution,or both) I also receive housing and council tax benefit.
Does anyone know whether I can get a free eye test and glasses as I am getting headaches above the eye and can`t afford to pay.
Also where would I stand with dentist charges I am in a N.H.S dentist and my check up is due, also what if I needed a prescription would I have to pay for that.

ESA paperwork is a nonsense. The Helpdesk told myself and my support worker that I was on IR but we eventually discovered that I wasn't, I was on CB. Ring them and ask for a call-back.
If you are on CB you may qualify for help from the NHS, you can ask the ESA helpline to send you an application form or pick one up from the job centre.Eat food. Not too much. Mostly plants - Michael Pollan
