    And does the second set state your premium has been increased because of your work at heights?

    bright grey send out a copy of the application form when they receive an online application with a copy of the initial quote attached, and in this pack there is a form which customers have to sign and return to confirm all the details are correct.

    Just because the pack you've received has questions answered about the work at heights doesn't mean the quote included in it will be based on this information as this is still subject to the application being underwriten.

    It sounds like your ifa has applied online, bright grey then sent out a copy of the app with the £15 initial quote included. After the application was underwritten they have increased the premium to £26 based on your work at heights and terms will have gone out to your ifa to say this.

    In the meantime you've contacted them to say you dont have a copy of the confirmation form and they've sent you out another one (which includes the pre underwriting quote).

    So basically unless they've received new information from your ifa to say that the work at heights info on the app was incorrect and they've removed the loading, your premium sounds like it will be £26. If I were you I'd be phoning my ifa to clarify.
