I received a bottle this morning as well - MDC. It came with the long initial survey I filled out, but no other information!
I just googled perfume and MDC - perhaps Miss Dior Cherie?! Will have to pop into a store and have a smell to compare! lol

Its my understanding that the perfume is sent out with the repeat questionaires so you complete a questionaire when you join and each year you complete it again thats when you are sent the perfume....my perfume arrived with my repeat questionaire this year.
