I would write a letter, send it by recorded delivery and state that the goods were made to order, to the specification agreed and as the client agreed to this when placing the order you expect full and final payment within 7 days or I will take further legal advice to recover the monies.
However, you are the one who knows how this is affecting your mother and if she really is being brought down by this, have to consider whether it is worth a detrimental impact on her health.
How much was the original item and what percentage refund is the client asking for ? At the end of the day, we can all give opinions of what we would do, but you (and your mum) are the ones who have to live with the consequences. Good luck x0
