ESA form advice please

Have received a form ESA50 today - looks terrifying:eek::eek:
It says on the front that you can get help from a relative or friend with completing the form, but then inside it says that if someone is completing the form on your behalf you have to fill in their details and say why - I am confused!
The form is for my husband, but there is no way that he would be able to sit down on his own and complete it, his memory is just not up to the task. So should I just go ahead and do my best with it? Any advice, tips, etc greatly appreciated.
Has anybody any experience of filling in these mega forms?

That is very helpful, thank you. My understanding from reading this is that a person needs to score 15 to get the benefit - is that correct?

Yes, to continue to get ESA, 15 points are needed.0

Have a read through the instructions and if you can fill it in for your husband that will be fine, just explain that he can't do it himself (a lot of people can't). If after you've read it you're still baffled then go get some help.0

Thanks, yes I think I will have to fill it in and so presumably will have to complete the box on page 2 stating that the form is being completed by somebody else.Stopped smoking 27/12/2007, but could start again at any time :eek:0
