As far as I know you just need a full license, the hire companies have their own insurance.
You may be asked if you want to buy 'collision damage waiver' as an extra, and depending on the price I think I would take it. It's usually to cover you for the large excess there will be on the policy and could be money well spent. Don't forget to check the van over for any damage before you sign for it because it will be checked over by them when you take it back.

Unfortunately I do not think a hire company will hire to you with your medical condition.
Your wife should have no problems hiring on her licence.
The insurance does not come in to it as you will have to take the insurance cover along with the hire.0 -
When ever I've hired vans I don't ever remember being asked about medical conditions.
I assumed they rely on the DVLC checks, ie if you are fit enough to have a car licence then you are fit enough to drive a van
There will also be an age limit, I'd guess it will be 21 or 250 -
....or above 75!0

I know that europcar have cars with special aids fitted for people with disabilities, not sure about vans but if you have a driving license I do not see a problem. Good advice from emmell regarding Collision Damage Waiver and checking the van very closely for dints and scratches before you drive off the rental station.0

Specially the windscreen, and any glassfibre on the sides or front if it's a luton. Insurance doesn't cover them, so make sure any marks are recorded before you collect it.0
