Help - template letter required!!!

Hi there
My partner had an accident at work a few years back, and due to a lot of messing around with doctors etc. well he left it too late to claim directly from his company.
Basically, he fell from height and now has permanent spinal cord and nerve damage, which he has had to have an operation to bring under control, but it cannot ever be fixed.
Industrial injuries did a medical and (given that my 35 year old partner walks with a stoop and drags his foot, is in constant agony and can barely move in a morning - but still goes to work every day and suffers the pain) they have decided that actually, he has no loss of faculty in any part of his body.
Needless to say, we're going to appeal, but I'm not the best at letter composition... can anyone give me any info to put into this letter, or direct me to a decent template letter?
We now only have 10 days to get the letter in - we'd been away and only just received the letter!!!

Thanks - to be fair they've already seen the medical reports, pre- and post-op letters and done a medical... they'd seen all the paperwork and decided he had a claim... the medical was to say how much he was affected... the doctor they made him see said there was no disability (which there is as he's registered disabled and they put us on to industrial injuries)... To be honest, I think the doctor's got his patients mixed up somewhere along the line!
Second opinion can't hurt eh!
