Help after Stroke

My Father is in hospital after having a stroke - had one about 8 years ago. He has a bungalow on a council run pensioners complex.
Anyway he is unsure of what benefites he gets (He is 73) As he has trouble walking he thinks he gets money for getting about and possibly help with cleaning.
A few questions - where can I find out exactly what he is getting?
Will he be reassesed in hospital - I know they are assesing his capabilities to feed himself and movement. A therapist called me to say that they will setup meals on wheels type service and 2 calls a day to make sure he is dressed / taking his medication etc - at present he gets no help.
secondly - will this tie in with the benefits he can claim - or do I have to investigate this?
What different benefits are available -( he couldnt drive )

    Can you get hold of one of his bank statements? If you can, it will show the regular payments in. Then you can take it to either your local Help the Aged, Age Concern or Citizens' Advice bureau and they will be able to 'translate' it for you and tell you what he might be entitled to in addition to what he already gets.

    You father should be able to claim Attendance allowance. This is only for care needs.

    I think what he is referring to is DLA that comes in a care and mobility part but can only be claimed by people under 65.

    Is there not a social worker on the ward? When my husband had his stroke nearly 4 years ago they had a social worker on the ward.
