Wow I've created a monster! Glad to know I'm not alone but think I'm going to stop shopping at Tesco and convert to Asda. At £2 (discount card) a visit, I'll never have to worry about last minute gifts for friends & family ;-)

It happens fairly regularly in Sainsburys with my Nana. She buys two bottles of whiskey every week and it has happened a few times that she has been charged for three. She is very old and gets tired so doesn't look at her receipt until she gets home and then someone has to go back and face them. There is no proof as the shopping is already home and they won't refund onto her card as she is not present. The one time I went back they kindly gave the refund on a giftcard. I think what happens is that they are the first items on the belt so they send one down, realise she's gonna need help packing so grab it back to put it in the bag at which point it scans again. The second one goes straight in a bag so only scans once. I'm sure the mistakes are genuine and probably not noticed at the time but yes, I always check my receipt before leaving the store.
