Holiday Insurance Recommendation

Can any one recommend reasonable single trip holiday insurance (2 weeks, Europe) which satisfies the following:
1. We are taking my daughters friend so the policy should include unrelated kids as a "family" group.
2. I have a prexisting condition for which I will make no claim but I don't want to get into lengthy discussions about it or have to produce doctors notes etc.
I found one that seemed to fit the bill but, unfortunately, I looked at dozens of insurance sites and didn't bookmark that one so I can't find it again.
