DLA 580 4 page form

Hello all, Just received my DLA renewal forms and its the new short 4 page version. I would much have prefered the full version to fill in. Has anyone here any experience in send back this new 4 page form?
I dont know if i should ring them and request they send me the full form instead in order to give full information. I've heard this new form is a con.
My DLA doesn't end untill mid July so i have plenty of time.

    There is nothing stopping you filling it in and adding pages to write additional information on, thats what i always do as even the larger forms arent enough for me;)
    But if you do this ALWAYS put your name and national insurance number on them and sign every page, then staple them to the form and make copies of everthing you send , that way if they should get lost you dont have to go to the trouble of doing it all again
  • what area are you from,I ask because i understand they are trialling this form in greater manchester,east lancashire and the north east,is that you?
    I have read mixed reports about the outcomes from these new forms but the majority are positive and have lead to a continuation of dla at same levels

    I just signed mine as nothing has changed and am now in the process of an appeal following a reduction in benefit. I would supply extra information and more importantly evidence from GP, consultants, support workers etc and send it in with the form - even though it does not encourage you to do so.

    ME/CFS , this is what i fear will happen. If you tick NO CHANGE then logic would suggest that you will get the same level award. Obviously not in your case. How can they justify your reduction in benefit when you state No Change? If No Change is stated then obviously you should receive the same award given using your previous application. Otherwise what is the point of this short form? Other than an opportunity for them to stop or reduce benefit .

    I had the 40 page form and there was no change, however I too have had a reduced care rate. I agree with you that if there isn't any change since your previous application how then can they justify a reduction - it doesn't make sense. I have written to them and awaiting a response!

    Thats what I thought. The form isn't very fair on the basis that if you tick 'no change' there is nothing on the form to advise providing information or evidence to prove there is no change. However I would provide as much evidence as you can to prove this is the case by asking your GP, Consultant etc to provide written evidence confirming your diagnosis and how it affects your mobility and care needs. I would do this asap because they can take a while to write back to you with this evidence.
  • I have just had my D.L.A. award redone under the appeals system, for reason not room here to go into, but over the years I have found that it is not what you say in your response to the questions it is how it is worded. I always get Welfare Rights assistance with this and we always get a good results even if the push it to appeal or even trubunal point . I feel this is because I am not alone in this and have someone with me who knows the ropes and what to challenge and when. I know many areas now have no longer got a W/R but perhaps the C.A.B. or many of the disability groups have assistance like the B.L.F. Good luck.
  • afaik most welfare rights are now attatched to your local council,as opposed in the past where they were just funded by them
