can't find extended warranty info!

Hi,
I have a samsung american fridge-freezer which has ice build up in it. (we recently had an engineer doing a "safety modification" on samsungs behalf, and he said our model doesn't usually have an issue with ice, but that the problem we have is a problem)
We had an issue last year, just before the standard 2 year warranty ran out, which was fixed, but now have more ice building up. Due to all the stories about samsung fridges, when Currys/coverplan wrote about the extended warranty OH and I decided to take one out, for a further 3 years, so I want to get them out to fix the problem. Only thing is I cannot find the paperwork
I have the direct debit still going out. Does anyone have contact details for coverplan?
