hi, i would like to tell the dla of changes in my care but do not want to phone,,do i need to get a new form to fill out or can i just email them..

Hi sh1305, was this a letter for the use of reconsideration on your award or was it to inform them of changes in your care/mobility needs within the allocated award period?
They specify on the award letter that you should contact them of any changes in your condition - was this the case? When you say you were sent a form - was it a different form to that of the application 40page form?

Wow sh 1305! So you had to fill in the whole of the form in again as if you were re-applying?! I thought when you informed them of any changes you just had to tell them what had changed and they would make a decision based on the information and your original form.
How long was it since your application and informing them of changes?0 -

What I meant was when you informed them of changes, I take it you informed changes in the middle of your allocated award - how far in? For example, say you had a 3 year award was it a year into the award that you had changes or ....It may have changed since I did mine. It was October 2008 when I informed them of change of circumstances.0 -

hi, i have about 9 months left of a 2 year award..i don,t think i want to tell them i,m getting worse if i have to fill in the form again ..that was a nightmare0

if you only have 9 months left i would leave it as you will get the forms in 3 months time to re-apply anyway0
