creating a form that needs to be filled in

i am wanting to create a form which asks for certain information such as name, address, date of birth, in total about 20 or so pieces of info.

i want to be able to print it out and have empty boxes for people to fill in, which vary in size eg address needs to be a bigger box.

would i be better off using a word program or spreadsheet program to do this?? i tried a spreadsheet and found that the writing to the left of a bigger row was at the bottom when i'd rather it be at the top. also it would mean i have to do the whole thing in one column as any row size changes affects information all the way accross the page.

tried in word and couldnt find how to make a box, tried using a table but i dont really want everything in boxes!! any ideas anyone? its just a basic form to collect information in a tidy way...

  • If you want to create a paper form for people to complete, use Word.

    You can use a Table to line things up neatly. Note: you can remove any or all lines from any cell in the table, and you can merge cells horizontally and vertically to have boxes spread across rows and columns. You can also change the alignment of text in a cell horizontally (left-centre-right) and vertically (top-middle-bottom). (This also applies to spreadsheet cells.)

    In Word if you turn on your Drawing toolbar you will find icons for creating Text Box which is a box in which you can type text, you can colour the border of the box or fill it with colour. (Search the help for Text Box if you can't find it - versions of Word differ where they put the icon for it)

    If you google for the type of form you want along with word template you'll probably find someone's already designed something similar, which you could use for ideas.
    A kind word lasts a minute, a skelped erse is sair for a day.

    Does it have to be paper based? There is a survey builder site I use which is an absolute god-send to me: http://www.surveymonkey.com/

    That said, theres nothing to stop you printing the surveys you create out :)

    Be careful of doing anything that asks for personal details online, if you are storing personal details online, then you need to register under the data protection act.
