Passport problem

Im trying to help a friend complete their passport. The problem we are having is that she has had a maiden name and a married name but is having to change her passport again due to the family name being changed by deed poll.
On section 2 it asks for maiden or all previos names (surnames first).
Does she have to complete that with all the names? Also does she have to send in birth cert, marriage cert and deed poll for all change of names?Also in section 3 it asks if person has had a previous passport and asks for the passport numbers to be included.
Does she have to do this for both her previous passports?

Finally does she require any countersignatures?

Any help would be much appreciated

Thanks that clears it up then. So basically they want any info on any previous passports and names.
As for the photo i think it looks pretty much like how she did 8 years ago so shouldnt be a problem there.

If she's up to 8 years on her passport why doesn't she leave it with her current name until it's due for renewal? If she changes it now she has to pay, then in two years time she will have to pay again for a renewal.

Any holidays can be booked in her passport name. It's all perfectly legal as you can call yourself what you want. Many women don't change their passports until they run out after getting married to save on cost/hassle.
