Search Facebook for Club Creative. They are pretty good usually at timely replies sent via fb message.

You can email / message them too via the official website, I did for my last one and they sorted it out really quickly.

Can't help with the prize list but as to cost you're looking at 3-4 stamps, 3 envelopes, a sheet of paper and some ink

PC have also been known to refund the cost of 1 stamp but unsure if that happens now/ every time.
