Problems switching to Halifax Reward Account

I have a Halifax Current Account and have twice signed a form to get it converted to a Reward Account but it never changed.
In mid July 2009 I went into my branch to pay in a cheque and while I was there I asked to switch my account and signed a form to do so. I checked my account online in October and saw that it had not switched to become a Reward Account.
So, in the first week of November I went to pay another cheque in and explained to the cashier what had happened and signed the form again. She said someone would call me that day to confirm it had switched. No one called.
Another three months on nothing has happened. I would like to claim back interest for the lost months if possible but I have no evidence that I have ever tried to switch. I didn't get copies of the forms
Has anyone had a similar experience, or can anyone suggest what I should do get it sorted out?

Call Telephone Banking on 08457203040 and they will change you over the phone - there's no form to sign. Once keyed the account should re-designate within 24 to 48 hours (best way to tell is via Online Banking, you're account name will change). Mention your on going issues you've had with branch and Telephone Banking may give a couple of fivers ex-gratia as goodwill for the poor service.Anything I post is my opinion, so from time to time I may be wrong.
