LPA template please?

I'm basically looking for a template LPA form, like you get for making wills, so i can avoid spending on a solicitor but still have a legally binding document at the end of it.
I think i'll probably take the 'homemade' LPA to a solicitor to be checked over anyways but the cost of this will obviously be less than them writting the whole thing.
So question is, is there a free template LPA document i can get my hands on?
I've read all the blurb so know what should be included, but for wording etc i thought it would be much easier to tailor a template?
