Have you tried the Allan Carr books? I only say because I sceptically got the audio book a few weeks back after being told they were brilliant but also quite a funny read. I never expected to quit smoking and I hadn't planned to do so.

I quit after listening to the audio book, barely suffered at all with cravings and haven't lit up since
