Questions about Amex cashback card

May I ask a couple of questions about the Amex cashback rewards card.

1) Do you need a perfect credit history. I have 2 missed payments in the last year & 2 recent searches - one for car insurance.
2) Do you need to pay by Direct Debit ? For my barclaycard I have a DD set up but normally pay in full prior to the due date so the DD ends up being nothing. Barclays reflect this but I understand that Amex may not work like this ?
3) I intent to use the card for expenses, is there a tax implication for the cashback ?

Also if you're thinking about applying, look on the referrals forum, as there's an extra £20 to be had if someone with a card refers you for one.
I've already referred a couple of people successfully.

Alternatively there's also £5 through Quidco, but you can't get both.
