We are planning a dream trip for OH's 30th bday and are looking at thailand as he wants adventure but I want to be safety conscious and would like adventure but also a bit of beach.
We'd like to do a twin/multi centre holiday - seems doable in 11 days or less.
Bangkok + Chiang Mai + Phuket would seem to combine city/adventure/beach quite well? Have also thought about bali or hong kong or both. Any suggestions, ideas, tips?? If any one has booked a similar holiday who did you book with? Whats the best value way of booking? thanks

Instead of Phuket take a look at the Krabi region. We booked our first Thailand trip through Kuoni who were fantastic, but since then have done DIY which works out cheaper.0

For only 11 days (nights?) I'd recommend 2 places not 3 as although doable, you'll be spending/wasting a lot of time on travel.

You can fly direct to each so you don't need to necessarily go via Bangkok, although it's a great place for say 3 nights. Phuket has lots to offer, and where you stay depends on what you are looking for - and also the time of year you want to go?
Patong Beach (south west corner) is by far the busiest and most popular spot (for sex tourism
). It's a bit like Thailands version of Benidorm
The next beach north is Kamala which is a lot quieter with a great beach, and only 15/20 minutes from Patong.
Chiang Mai is great, and has a nice mix of old and new. It has a large night market and some nice restaurants. Can't remember the name of my favourite, but of you pm me I'll find and send you a link). There are also elephant farms in the jungle around the city whci are well worth a visit.
There are loads of threads on here with a lot more info and also some very good forums on the net with real up-to-the-minute help and advice.

I would also recommend sticking to 2 places with only 11 days. I didn't think that much of Phuket and would also recommend the Krabi area or Koh Samui.
Hong Kong is fabulous but as you say you are safety conscious Bali might not be your best option as the Ausralian Foreign Office advice is currently to reconsider your need to travel to Indonesia including Bali.0
