DPS Documentation - where can I find copies?

Where can I find copies of the following documentation please:

Joint Deposit Repayment Form
Statutory Declaration Notice Form

Without having to contact the DPS. I only want them at this stage for info and they are saying they won't email them to me (blank ones!!) without further tenancy info which I do not wish to provide at this stage!!

I can see dealing with them is going to be an issue. Isn't that sort of documentation meant to be available freely as per the Freedom of Information Act 2000....

    you can get a Stat Dec form by googling - and then fill in it, provide the supporting documentary evidence you will be submitting to prove your case, and then take it to a Solicitor and for £5 he will "swear" it for you - there is no standard format for a stat dec

    do a search on here for "Deposit Protection Service representatiave and ask him directly on the thread he is currently responding to..... the thread is called ""Royal Mail could cost me £2000! Please help""

    Don't suppose you have a copy of the JDR form for the DPS do you?

    ETA - I emailed the DPS saying that I felt the fact they would not provide the blank forms without further information was against the principles of the FIA 2000 - this sort of info should be in the public domain. Certainly the TDS (which on paper to me, seems a better scheme!), have their forms online for anyone to see!
