Financial hardship question

My boyfriend is on a Debt Management Plan. He is not on high wages and the majority of what is left each month goes straight to the DMP.
One of the creditors that he pays is Lloyds and is actually an overdraft - it started off low but each month it incurrs spiralling charges and is now in its thousands. Despite him being on a DMP they have not stopped or frozen the charges and thus he pays off less every month than he is charged.
Would this qualify for a financial hardship charge reclaim? If so, how would I go about helping him to do this?

Who set up the DMP?
Did Lloyds accept it?
Was there any agreement to freeze interest or other charges?
(To be honest, it would be a bit of a poor DMP if the agreement was to pay something that didn't even cover the ongoing charges if they were still to be applied)
