Shower cartridge help please

I am having problems with my shower and after reading some other threads I think the cartridge may be at fault. I've recently moved to a new area and dont know a plumber here and quite fancy having a pop at fixing it myself - any advice would be really appreciated.
Some info - the flow of water in the shower is really poor when the shower head is raised - only very hot water coming out in a trickle. When put near the ground water flow improves but still not great. Our other shower has been working reasonably well.
As far as I know both showers are mains fed and have a mixer valve (?) - Reliance 153 HC . Boiler is an ideal classic rs260. We dont have instant hot water.
I've tried removing the cap of the mixer and can see a screw there. When I take this out, water comes out but doesn't spray out or anything. havent gone any further yet as I'm not sure what I'm doing!!!!:o

The cartridge ,,,,,,, can be replaced, or new washers and springs fitted, However somethimes cheaper to get a new mixer shower unit.0
