My life Your stories competition

1. This competition takes the form of a Prize Draw for approved entries.
2. You must be between 12 and 18 years old to enter this Prize Draw. If you give false details you will be disqualified.
3. Entrants who are under 16 must obtain prior permission from their parent or guardian before entering the prize draw.
4. This Prize Draw is open to all residents of the UK and ROI except employees or contractors of Diabetes UK or Specsavers and their families, their agents or anyone else connected with the Prize Draw.
5. The competition opens on 10 November 2009 and runs until 15 January 2010.
6. By taking part in the Prize Draw, you automatically agree to the Terms and conditions that apply for this Prize Draw.
7. Suitable entries will be selected for publication on Diabetes UK websites by the Diabetes UK Digital Media team. By sending us your story, you agree that Diabetes UK can use and publish the stories in this way, and in any other media to promote the Prize Draw and websites.
8. Diabetes UK’s decision is final. Diabetes UK is allowed to disqualify any entry at its own discretion. No correspondence will be entered into regarding such decisions. All decisions issued by Diabetes UK in respect of this Prize Draw shall be final and shall not be subject to appeal.
9. Diabetes UK is allowed to exclude submissions that it believes to be fraudulent.
10. For each story approved/selected by Diabetes UK, which includes valid contact details, the entrant will have a chance to win the Prize Draw prize.
11. The winners will be drawn at random in a prize draw from all approved/selected entries.
12. All submitted written material will be checked and edited by Diabetes UK to make sure it is appropriate before it is uploaded onto any websites.
13. One winner of the ‘My life with diabetes’ teens’ story Prize Draw will win a Nintendo DS Lite with Nintendogs game, and a Nintendogs Puppy Playmate. Two runners up will each win a Family ticket for Drayton Manor Theme Park.
14. No cash alternatives or other alternative prizes are available.

16. The winner will be notified by email after the closing date.
17. The Drayton Manor theme park family ticket is valid from 1 Jan 2010 until 30 November 2010, excluding Bank Holiday Sundays and Mondays. A family ticket is for four individuals. The ticket must be presented at the ticket booth prior to entry into the park. No cash refund or alternative is available. It cannot be used in conjunction with any other offer or promotion. Originals only accepted – no photocopies/ damaged/defaced/altered tickets.
18. The winner may be asked to take part in publicity.
19. No responsibility can be accepted for entries lost, delayed or corrupted when uploaded online or emailed. Proof of entry does not constitute proof of receipt.
20. Diabetes UK reserves the right to request written proof of residency or of age of any winner before the prize is dispatched.
21. The address details submitted by the winner are deemed to have been submitted by the authorised user of the email address. Diabetes UK will store the provided information for the purposes of carrying out this promotion.
22. Diabetes UK is not responsible for network, computer hardware or software jamming or outages of any nature which restrict, slow down or cause the loss or the entrant's form or other data.

24. Diabetes UK retains the right to alter the present terms of entry in the interests of the entrant, with no less than 28 days’ notice, to be posted on the website.
