Who to contact for error on credit file?

I've just paid £2 to see my statutory credit report on Equifax, and there is a late payment shown for an account which O2 cocked up when closing it down under the 14 day period they allow (brand new account, unused, closed within 14 days). They didn't close it down correctly (as they admitted on the phone) and as such, the 14 days elapsed and I ended up in arreas totalling £14. They even sold the debt straight onto a debt collection agency, as I had a letter from Westcott demanding the amount. What is the procedure for having this removed from my credit file? Who do I need to contant?
