Compenstation for late train termanated short (Paris to Belin)

First the train was 80 minutes then after about 4 hours travelling it broke down and was stationary for about 5 hours, then after another hour it was terminated early and we had to spend 5 hour on a train which, most of the time was standing room only, arriving 4.5 hours late.
Compensation forms were handed out, but I don't understand them as there are in German which I cant read. The form does refer to English translation online - but it isn't there. Given that the train is question is the Paris to Hamburg/Moscow/Berlin and they use English when selling tickets that's rather poor customer service.
I will go Berlin HBF station for assistance tomorrow, but does anyone have any other hints?
