Bands for NHS Glasses Voucher

Hi all i understand the different amounts for voucher types for glasses, mine was Voucher A currently standing at £33.70, however we have been trying to get my mum to go to the optometrists for a long time now, she got her last pair i think over 15 years ago and she is extremely short sighted and her sight has got worse, her current lenses i would say are 3-4 times thicker than mine and i am -4.00
My question is when do you start to be awarded the higher voucher bands? Is it down to strength or other factors? I aksed my optom but they were very vague about it. She is worrying about cost as her lenses last time cost an absolute fortune and she only got voucher A which at the time was about £15, my dad gets PC(G) so will be entitled to full voucher.
Can anyone advise who has either got a higher voucher or got very bad eyesight?
Kind regards

Ive always understood that the vouchers are paid by virtue of the quality of your eyesight only, the worse the eyesight the higher the category.
The voucher values can be found in form HC12, here

My views are my own reading of the law and you should always check with the local authority in question.0 -
Thank you CIS i have a HC12 here just a shame it doesn't give any indication as to what diopter either - or + moves you into a higher band. I suppose we will just have to wait and see! LOL!0
