The appointment, as stated in a previous post, is to contract your Job Seekers Agreement. The confirmation of details is just that.

If you are applying for contribution based JSA you do not need to answer the financial questions. The application is then sent away for assesment and sometime later (about a week?) you will receive notification of acceptance or rejection of your claim. If your application is rejected you then need to reapply for income based and therefore complete that part of the form.

If the second application is accepted it will only take effect from the date you completed the second form and will not be back dated. It is important therefore that, if you are applying for contribution based, that you are absolutely certain that you qualify. If there is any doubt you should fill in all of the form accurately then if your contribution based claim fails they will automatically process the income based claim.
