Notice of maternity leave - what to put in letter to employer?

Hi,

As I am 25 wks pregnant and I need to produce a letter for my employer stating that I am pregnant and intend to go maternity leave, however I not sure just what I need to write. Could someone please give me an idea of what my letter should contain?

    Hi firstly congrats on yr pregnancy. Last year I had to write this letter and mainly just had to state that I planned to commence my maternity leave on whatever the date and if I felt I could not manage until this date would inform them asap. Also the midwife will give you a MAT B1 form which is a maternity certificate, yr employers will want to see this too.

    Initially you will need to just write a short letter confirming your pregnancy and when your baby is due so that they can start their paperwork. They should on receipt of your confirmation provide you with their maternity policy and usually provide a maternity risk assessment. They will also tell you when your MAT B1 is due. They will also tell you whether you are entitled to ordinary leave or additonal leave. There is agreat website which I can't think of the full name off the top of my head but it's a government one, called TIGRE you could ask Jeeves/Google and they would list it. Congratulations by the way.
