Just got a P45 - still employed?!?!

I've got up this morning to the post, to find the three part P45 on the mat in a DWP envelope. There were just the three sheets of paper making up the P45 and a black page with the address - thats it, no explanation, nothing.
So I have looked through and its got some stuff on it that doesn''t make sense - the biggest bit that doesn't make sense is that I am still employed by my employer! I have, more to the point been paid 4 times since the date I apparently left employment.
The three page forms have no pay figures on them, detail a leaving date of 04/08/2009 - there's no mention of my current employer on the paperwork either.
Then it gets odder, the Jobcenter Plus address shown is Halifax BDC, Leeds - yet I live in Leicester?!?!
This has really upset me this morning, I have no idea whats going on, but I now have to spend the whole weekend worrying about whats going on.
I called HMRC online team this morning to ask them - all I got back was that your employer has requested this - you will need to speak to your employer - I can, but not till Monday - I'm panicing
Anyone have any ideas what the heck's going on?

I have never claimed a single benefit for anything...its going to the point where, given I am in Leic and the job center listed on the P45 is in Leeds we wonder if my details are been cloned/stolen.
But I have no idea where to turn to?0 -
Unfortunately you have no option other than to take up the matter with your previous employer because it is them who generated your P45. Good luck . . .0

but I dont have a previous employer - I have had self assessment roles and this job, I have worked there my whole career.

Could it have been sent out to the wrong person with the same name?0

all the names and national insurance number etc etc are all right -and apparently you dont get these when you self assess - I can only think its someone whos using my details for something0

Halifax BDC is a Benefits Delivery Centre, the P45 would normally be sent out when someone ends a benefit claim. Try to give them a call - there may be someone there until midday.Gone ... or have I?0

I doubt that your employer would send out a P45 statement if you are already working there unless it was a mistake. However you have stated that the employer's details is not on the statement. Strange.0
