NHS Continuing Healthcare For Severe Dementia DST Eligibility Requirements

Hi We are having the first full NHS Continuing healthcare assessment on my mother-in-law who has severe dementia next week. I see the assessment invloves filling in a DSt (Decision support tool) which has 12 domains scored at 6 levels (no need through to priority need although some domains no not have the top 2 levels)
I see the checklist indicates elgilbility for NHS continuing hwalthcare with either
1) the top level (priority) in any one domain or
2) the next higiest level (severe) inany two domains or
3)one domain at severe and other needs in a number of ohter domains
4) a number of domains wiht high or moderate needs.
Whilst eligibility wiht (1) and (2) are quite straight forward eeligibility through 3 and 4 seem a bit woolly and depend on what 'a number' actually means.
I was wondering if any one who had gone throuhg this process for an individual with severe dementia could post details of what they scored in each domain and wherever this was a pass or fail?
I'll post our details here when we get them.

In the end we were successful in our attempts to get CHC for our mum for severe vasuclar dementia so will when time allows but details of this on the site above in case it is useful to others.0 -
