Going from SSP to Incapacity Benefit

Hiya, Don't know if anybody can give me any advice....

I am currently receiving SSP from my company on a monthly basis, However I know that you can only claim this for 28 weeks maximum.

Am I right in thinking that after this time I have to apply for Incapacity benefit?

What I would really like to know is will this happen automatically or do I have to contact my local benefits office? I've only got just over a week left before this timescale is up and Don't know what else to do.

Sorry to go on a bit but this all a bit confusing to me! Any help will be REALLY appriciated! Thanks Peeps!
