I think it would be perfectly reasonable to ask what their concerns are and get your doctor to respond with only the relevant information. There should be no need for them to see evrything if it is not relevant to the relationship between you and your DD.Taking responsibility one penny at a time!0

Could your ex have got wind of the issue you are trying to hide? and be behind the request? I know of a case where the details of someones medical history came out because of a chance/unguarded remark by the Receptionist at the surgery, probably very unusual, but still possible.0

Yep, that’s what countless others have said, the big question is WHO decides what is relevant and in reality that’s going to be an officer of the court (judge, doctor appointed by judge, or welfare officer) and not the OP or her GP.
As LazyDaisy advised, if it’s a fishing request by the ex’s solicitors (which it sounds like) rather than an order from the court then the OP can refuse to disclose anything she is uncomfortable about, or indeed anything at all.0
