    Exactly what I said....

    No you do not need to pay anything. It is your file. You have (presumably) paid for it. The solicitors have the right to remove internal notes, etc, but you are entitled to the file itself.

    The SAR would only cover notes stored in an electronic form. Most Solicitors only keep 'hard' files for obvious reasons.

    That's not correct. The 1998 Data Protection Act extended the definition of personal data considerably. Non-electronic notes are also covered.
