Where Can I Get Double Ended Phone Leads?

I have got a surge protector that can protect the phone line as well as the electricity supply. I now need a lead with a connection at one end to go into the BT box on the wall and an identical connection at the other end for the surge protector. Does anyone know where I can get one of these?

    I'm sure
    you can buy these from large DIY stores like B&Q
    but if you you can buy the BT ends and make the cable up yourself
    pins 2,3,4,5 are theonly ones to connect
