where to send hc1 form???

i have filled in a form called hc1 for help with dental work etc, but i dont know where to send it does anyone know where???

Yes, it's for everyone.

When you get the HC1 form, there will be a prepaid envelope provided somewhere inside the form. It's strange that you didn't have one with your form.
