Reclaiming higher-rate tax - which form

I've put this off very late this year, as I've been waiting for details of my mileage claims from my ex-employer, which I stupidly forgot to print copies of before I left!
The information arrived today, and I now realise that I don't have the details of the form I filled in last year to provide HMRC with information on
if anyone knows which form I should be completing (hopefully it is one of the ones that is available online) I'd really appreciate it and will keep the information on record in future!

- mileage claims (claiming back tax on the difference between the 40p/mile allowed and the 12p/mile I was getting)
- savings income
- contributions to stakeholder pension
- income from shares
- charitable donations
if anyone knows which form I should be completing (hopefully it is one of the ones that is available online) I'd really appreciate it and will keep the information on record in future!

It sounds to me as you should be completing a Self Assessment Tax Return to me. I cannot think of another form which can include all of the details you have mentioned.0

Well they definitely sent me one last year - apparently it didn't merit sending in a full return (mind you, they still managed to get it wrong!). I may have to complete a full return in order to get it in on time anyway.0 -

The form sounds like a P810, which is not being issued this year due to the new computer system being implemented accross the country.
To ensure that you do not need to complete a tax return, can you confirm that:
Your claim for expenses does not total £2500 or more?
Your income from interest and dividends is less than £10000?
Is your total income less than £100,000
If the answer to any of these is NO, then you must do a self assessment.
You may be able to give these details over the phone if your expenses claim, gross pension contributions, total interest/dividends, and Gift Aid payments are less than £2500 each then assuming you can confirm your employment details for the last tax year, an advisor can take this information over the phone and arrange for an assessment to be completed.
If any of these are above the £2500 limit, but below the self assessment limits,then you can simply write to the tax office, breaking down each element, and request an assessment is completed.

Thanks! I thought I was going mad... and halfway through filling in a self assessment form, while looking for my charity info (which clearly I filed somewhere different last year
) I found the guidance notes for P810. I am below all the thresholds you mention (the claim is pitiful... but I'm not going to let the taxman have anything he isn't entitled to just because I can't be bothered to do the paperwork!), but as I'm now nearly all the way through, I'm going to persevere so that I have all the calculations worked out. I'll probably have to submit the form online as I'm unlikely to find the time to call anyone...!
