Questions on cleaning up credit file

Having read the IVA and Bankrupcy clean up 'stickies', I'm a little unsure about some of the mechanics, so here are the questions -

1) The BR letter seems to imply that clean up can only start after discharge. What is the situation for IVA? Can it start while the IVA is underway?

2) In our own case the IVA failed, and we expected to be made BR, but our creditors have decided that they just want the equty from our house once it's sold (and that might take a while in the present market :rotfl:). So now the IVA has been terminated. I presume, notwithstanding the above, that we can start the cleanup process right now?

3) For BR, both Bankruptcy Order and Discharge Notification are included with the letter to the creditor, what documentation, if any, should be included with respect to IVAs?

4) Is there anything else I need to know?
