Requesting an application form via email

When I've been searching online and in the local newspaper for jobs, I've noticed that some organisations don't have the facilities allow you to apply online so they ask you to apply for an application form via email or telephone.
I've seen a job advert which I like the sound of, but it asks you to either email or telephone them for an application form. There isn't enough information in the advert for me to tell whether I'd definitely be suitable for the job, as it doesn't have a list of qualifications or experience required.
I'd prefer to request an application form via email, as I'd be less nervous than I would be phoning them. If I send an email requesting an application form, what do I need to say? Is there a template I can use for my email? Is it advisable to include information in the email about why I want the job and a brief description of my skills, or is it best to just say I am requesting an application pack and not write anything else?

Unless they have asked for anything specific, I would just supply the details they need in order to send you a form, as I guess there will be a space on it to sell yourself.0

It just needs to be direct to the point. It will be an admin person reading it - all they need is the job you are referring to, and somewhere to send the details to.

Jadziad has pretty much covered it. One thing to add - don't stress about asking for the form or about ringing up to ask for it. Such Emails/calls are read and dealt with by junior admin staff who probably get hundreds each month and delete them as soon as the forms have been sent out.0
