HC5 claim form - mileage allowance?

I recently had to make a 100+ round trip to hospital for treatment.
I realised later that I can claim help with my travel costs so have got a HC5 form to complete for a refund of my petrol costs.
All the form asks for is how much I want to claim. Does anyone know the mileage rates paid please as I don't know what figure to enter. I don't want to over/under claim?
I've got a petrol receipt to send with the form but that was for filling the tank and not all was used.

Each hospital / Trust set their own rates. Mine vary from 11p/mile (with no reimbursement for parking) to 35p/mile (plus parking reimbursed).If you receive full help with prescriptions, specs etc. can you reclaim hospital travelling expenses?
The Healthcare Travel Costs Scheme (HTCS) is for patients who do not have a medical need for ambulance transport and who cannot afford the cost of travel.
You may be entitled to HTCS if you or your partner get any of these benefits:- Income Support
- income-based Jobseeker's Allowance
- income-related Employment and Support Allowance
- the Guarantee Credit part of Pension Credit
